An individual survey can be used for two purposes: To record an individual score that was received prior to the emailing of the survey or to email one single survey as opposed to uploading an entire CSV list.

1.  Click Survey tab on the left side menu.

2.  Click the drop down "Send a Survey".

3.  Select "Create Individual Survey".


4.  Fill in all fields that are applicable.



5.  Survey Type:

a.  Email - Choose email if you are going to email a survey to a client. 

b.  Phone - Choose "phone" if you want to enter a score and/or comments from a phone call (inbound or outbound phone call)

c.  QA - Choose "qa" if you want to enter a score and/or comments from a QA check.  If you want to separate the scores and comments of a team to show individual results, just enter one QA result per individual team member.

6.  The results section will only show if you are entering a "phone" or "qa" survey result.
